Eligibility Requirements
To be eligible to choose a plan offered by Samaritan Advantage Health Plan you must meet 4 requirements. These requirements are:
  1. You are 65 or older or have a disability and eligible for Medicare
  2. You have Medicare Parts A and Parts B and you continue to pay the premium. For most of you this amount is already taken out of your Social Security before your Social Security check is sent to you.
  3. Your permanent residence is located within our service area being Linn, Benton and Lincoln County. AND
  4. You have not currently been diagnosed as having ESRD (End Stage Renal Disease) also known as kidney failure.

Beneficiaries of Samaritan Advantage Health Plan must have Medicare Part A and Part B and must continue to pay Medicare premiums.
Enrolled beneficiaries must use Samaritan Advantage health Plan contracted providers for routine care.
Samaritan Advantage Health Plan is an HMO with a Medicare Advantage Contract.
