You can get a detailed description of your plan in the Evidence of Coverage including your rights and responsibilities as a member and the grievances and appeals processes. You can also view the covered classes that are offered to you from Samaritan Health Services as a member of the community. All of these options are available to you by clicking on the specific buttons on the left of this page.
